Entropion and ectropion are basically opposite problems with the same part of the eye, the eyelid margin just isn't sitting where it should. In entropion, the lid rolls inward toward the eyeball, so the lashes end up rubbing against the eye itself, which is about as uncomfortable as it sounds. Ectropion's the flip side, the lid rolls outward away from the eye, leaving the inner surface exposed and usually pretty irritated. Both mess with how the eye's protected day to day, and neither one tends to get better without some kind of intervention once it's actually causing problems.

Mild cases sometimes get managed without surgery first, using lubricating drops or ointment to protect the eye's surface while a doctor monitors whether things are getting worse. This buys time but doesn't fix the underlying muscle laxity causing the problem.
Persistent irritation, redness, or a feeling that something's in the eye that doesn't improve with drops is usually the point where surgery becomes the more practical option, since these symptoms mean the eye's actual surface is under ongoing stress.
